DPS MEMBERSHIP APPLICATION

There are FOUR steps to joining or renewing your membership.

STEP 1: Fill out the entire form below if you are a New Member and complete the four steps.

If you are renewing, please select the option "RENEWAL" and fill in your name and address and complete Steps 2, 3 and 4.

If you are Updating your information (i.e. moved, new phone number) select the Information Update check box, fill in your name, address, and information that needs updating and click on Submit Form in Step 2.

STEP 2: Click on Submit Form. This will take you to a confirmation page.
STEP 3: This is a confirmation page in case you want to print it out. Click "Return to form" at the bottom of the confirmation page.
(Please note: "Step 3" is not specifically mentioned on this page.)
STEP 4: Click on PAY DUES. This will take you to a PayPal website for completing your payment of Membership dues.
